Afro-textured hair requires a different extraction technique, and a surgeon who has performed enough of these cases to know it.

The follicle of Afro-textured hair curves beneath the skin. A punch aimed straight down at the visible hair shaft will frequently transect the follicle below the surface, destroying the graft. Standard FUE technique applied without adjustment produces high transection rates and poor yield.

How the technique adapts

• Larger punch diameters to accommodate the subsurface curl
• Slower, more deliberate extraction with frequent depth adjustment
• Careful assessment of curl pattern before the session begins
• Reduced grafts per hour, meaning longer sessions for the same count
• Attention to keloid and hypertrophic scarring risk in predisposed patients

Where Afro-textured hair has an advantage

Once extracted successfully, the results can be excellent. Curly hair provides greater apparent coverage per graft than straight hair because each hair occupies more visual space. Men with Afro-textured hair often achieve a convincing appearance of density with fewer grafts than a straight-haired patient would need for the same area.

Scarring considerations

Patients of African and Afro-Caribbean heritage have a higher baseline risk of keloid and hypertrophic scarring. We assess this at consultation, ask about any previous scarring history, and where appropriate perform a small test area before committing to a full session.
